If your student needs to take prescription medication during the school day, please have your doctor or clinic complete the attached Medication Authorization form. If you would like your student to have permission to occasionally take over the counter medication such as ibuprofen or seasonal allergy medication, please complete the parent portion attached Medication Authorization form.
